this is very recent, here's a capture from monday: web.archive.org/web/2018091013

this page now exists and is titled "sex designation change"... travel.state.gov/content/trave

*there are language changes to the required letter*, but your doctor still determines what is "appropriate clinical treatment" and the page still says surgery is not a requirement
