The center is a timeline of the Real. from left to right, the Past, current situation, coming challenges, guidance, and likely outcome.

Above that is what is Known, below that is what is surrounding and Obvious.
One card crosses the current situation in the center left. It isn't read up or down, it's Both.

The left side of the known and the surrounding is what is already known by the person receiving the reading. It's what pertains to the central conflict and the current situation. On the top right, you've got what is Known, but to everyone But you, and below that, you've got what's all around you but you can't see it.

It's a useful spread because it can be expanded as needed if a certain aspect needs more explaination.
